Mattresses for Back & Joint Pain
The core of a sleep system for a bad back — memory foam and hybrids compared on firmness, support and the certifications each one actually carries.
The mattress is the foundation of a sleep system for a bad back — get it wrong and no pillow or topper will save you. But shopping for one is a mess of testing claims you can’t verify and hidden specs. We cut through it two ways: we only feature beds you can actually buy on Amazon today, and we lead with the thing the big review sites skip — the certifications each mattress states, and what they really cover.
Below you’ll find our pillar roundup for back pain, targeted picks for lower-back pain, side sleepers and heavier bodies, and full reviews of the beds we keep coming back to. Every pick pairs a pain-relief use case with a named, checkable certification.

How to choose a mattress for back pain
Start with firmness, because it’s the setting most tied to the evidence. For most people with back pain the target is medium-firm: a well-known randomized trial in The Lancetfound medium-firm mattresses beat firm ones for low-back pain and disability. Too soft and your hips sink so the spine sags; too firm and your shoulder and hip take the pressure. Your body weight nudges the target — heavier bodies want a touch firmer and more supportive, lighter bodies a touch softer.
Foam or hybrid?
The second decision is construction. All-foam beds contour and relieve pressure, which suits lighter and side sleepers. Hybrids add pocket coils for support, airflow and edge strength, which suits heavier bodies, back sleepers and couples. Neither is universally “better for the back” — it depends on your body and how you sleep. Our memory foam vs hybrid guide walks through it.
The mistake buyers make
Buying on a scary word or a reassuring logo without checking the scope. A “CertiPUR-US” badge only certifies the foam — it says nothing about fiberglass in the fire barrier. If material safety matters to you, look for a stated fiberglass-free build and read our Certification Decoder before you spend.
Frequently asked questions
What firmness is best for a bad back?
Medium-firm for most people, based on the Kovacs Lancet trial and later reviews. Heavier bodies can go a step firmer for support; lighter and side sleepers a step softer for pressure relief.
Is a mattress in a box any good for back pain?
Yes — most of the certified, back-supportive beds we feature ship compressed in a box. The format is about shipping, not quality; what matters is the firmness, construction and certifications inside.
How do I know a mattress is fiberglass-free?
Check the maker's own statement. We mark a bed fiberglass-free only when its listing states it, and our fiberglass-free roundup lists the ones that do.
